Effective March 15 - all those eligible for a booster shot must show proof of receiving that third booster shot/dose of the COVID-19 vaccine to be considered fully vaccinated. Exceptions to this rule may be made by the professional team on a case-by-case basis for individuals who cannot be vaccinated for medical reasons. We are asking that all members of Congregation Beth El please fill out the following form -- those who have been boosted and those who are not yet eligiible to be boosted. We will keep this information in our records and data base. Upon receiving confirmation, you will be mailed a sticker to be added to your "Beth El Go" access card to show upon entering the building which confirms that you are fully vaccinated and boosted and allows for entry into our facility. 

Please note that this registration form must be filled out separately for each individual in your household that is fully vaccinated. Please call our synagogue office at 973-763-0111 with any questions you might have.
